Eye Examination Letters

Here are prewritten letters that recommend or require eye exams.  The Vision First preschool or student report form should be sent with each letter.

Important note

The Kids Eyes Count Campaign needs to be approved by your school or school district before materials are distributed. 

Please read the Proposal for Schools and follow the A+ Action Plan for Schools.  We hope you will find these letters helpful, informative, and easy to use.

 

Here are simple directions

  • Choose preschool, kindergarten, grade school, or high school.
  • Choose the recommended or required examination letter.
  • Insert the date, your name, your school, the school’s phone number, and approval information in the blue/gray sections.
  • Print your copies. 
  • Distribute the letters and attachments as shown in the A+ Action Plan for Schools.  We made it simple for you.  Each letter has its name in the lower left corner with the name of the attachments included. 

 

Check your printer's setting

For full size copies, make sure your printer's print setting for "scale to paper size" is set for "no scaling."
