Prewritten letters

The Vision First prewritten letters make it easy for schools to participate in the Kids Eyes Count Campaign.

Follow these simple instructions:

  • Choose eye examination, vision screening, or failed referral.
  • Choose preschool, kindergarten, grade school, or high school.
  • Insert the date, your name, your school, the school’s phone number, and the approval information in the blue/gray sections.

  • Print your copies.

  • Distribute the letters and attachments as shown in the A+ Action Plan for Schools.  For easy identification, each prewritten letter has its name in the lower-left corner.  The name of the attachment is included.

Important note

The Kids Eyes Count Campaign needs to be approved by your school or school district before materials are distributed.

For further information regarding your school’s participation in the campaign, please review the Proposal for Schools.
